The lower eyelid was addressed by repositioning fat rather than removing it. In patients whose hollow sits immediately below a bulge, taking the fat away deepens the groove and worsens the shadow, so the Hamra technique moves that same fat down over the orbital rim to fill the depression. Access was through the inner surface of the lid, leaving no external incision. This image is three months after surgery, by which point the repositioned tissue has softened and settled into place. Slight firmness is expected during the first couple of months and is not a sign that the position has shifted.

Risks & Side Effects

Discoloration and swelling around the lower lids are marked in the first week, ease over two weeks, and can take longer to disappear completely. Tightness, dryness, watering, or numbness of the cheek may continue for a month or more. Residual shadow, uneven contour, or a small remaining bulge occasionally needs a second procedure. Rounding or outward turning of the lid margin is uncommon. Bleeding, infection, and slow wound healing are unusual.
