The Hamra technique was selected rather than simple fat removal because the shadow under the eye was caused by a step between the lid and the cheek, not by excess fat alone. In this operation the fat is repositioned downward over the orbital rim to fill the groove, which flattens the transition instead of hollowing it further. The approach preserves volume that a straightforward excision would discard. This photograph is one month after surgery. Bruising has cleared and the outline is visible, though slight firmness along the repositioned tissue is still present at this stage and softens over the following two to three months.

Risks & Side Effects

Swelling and bruising of the lower lids and cheeks are pronounced for about two weeks and can take a month to resolve fully. Numbness, dryness, watering, and a tight feeling are common early on. Uneven contour between the two sides, residual hollowing, or a remaining bulge may require revision. Pulling down of the lid margin is uncommon and usually temporary. The incision heals as a fine mark inside the lid or beneath the lashes. Bleeding and infection are unusual.
