A hybrid approach was used because an implant alone would have left a visible edge at the upper border given this patient’s tissue thickness. A 250cc Motiva implant provides the underlying shape, and 65cc of the patient’s own purified fat was layered over the upper pole and cleavage to soften the transition between implant and chest wall. The photograph is six months after surgery, late enough that the retained fat volume is stable and the implant has fully settled. A proportion of grafted fat is always absorbed in the first months, which is accounted for when deciding how much to place at the time of surgery.

An implant placed through a small incision brings tightness across the chest, swelling, and pain on raising the arms for one to two weeks. Bleeding, hematoma, or infection may require a further operation and, in some cases, removal of the device. Capsular contracture, rippling, rotation or displacement, altered nipple sensation, and rupture over time are recognized. Revision at some point is possible. Late swelling of one breast has to be assessed, since a lymphoma linked to breast implants has very rarely been reported. Radiology staff must be told before mammography.

The grafted breast and the areas from which fat was harvested stay swollen and bruised for about two weeks, and compression is worn on the donor sites. A substantial share of the injected volume is reabsorbed within three months, so the early appearance is not the endpoint. Oil cysts, calcification that can show on imaging, firm lumps, and uneven contour may follow. Infection is uncommon and a second grafting session is sometimes discussed.
