Treating the nose, the alar base and the folds together is deliberate. Correcting the fold alone tends to flatten the transition into the cheek and the line returns quickly, because the cause sits higher up where the base of the nose has lost its support. Filler along the nasal dorsum and at the alar base is placed deep and in the midline, since the vessels supplying this region have limited collateral flow. The chin was built last, once the upper structure was set, so the profile could be judged as a whole. This photograph is immediately after the session.

Risks & Side Effects

Filler in the nose carries the highest vascular risk of any injection site, and either intravascular placement or pressure from the volume used can compromise the blood supply to the skin. Blanching, mottling or pain out of proportion during the session is treated as an emergency and hyaluronidase is kept immediately available. The nasolabial territory carries a smaller but real risk for the same reason. Bruising and swelling at four separate sites is common and settles over one to two weeks. Lumps, asymmetry and gradual loss of correction over nine to eighteen months are all possible, and hyaluronic acid can be dissolved if the result needs adjusting.
