Alar Reduction narrows the width of the nostril base and reduces outward flaring for a more balanced lower nose. Two approaches are available: the internal method, which narrows the alar base from inside the nostril with virtually no visible scarring, and the external method, which excises the outer alar skin to address outward flaring. Both techniques are designed to minimize scarring. The two methods may also be combined depending on each patient's nasal structure. The Flap Technique is an advanced alar reduction technique that preserves the soft tissue instead of removing it entirely. The skin is excised while the underlying soft tissue is kept intact, shaped into a flap, and drawn through a subcutaneous tunnel to the opposite side, where it is sutured in place. For patients whose nostrils flare when they smile, this technique helps pull the alar base inward for a more controlled lower-nose shape. Alar Elevation is a surgical procedure designed for those with drooping alar (nasal wings) that extend below the columella. Excess skin and tissue are removed along the natural groove of the alar, and the alar is repositioned upward and secured in place. Alar Base Augmentation, often referred to in Korean aesthetic surgery as “Noble Surgery,” — is a procedure that lifts and volumizes the recessed alar base, the area at the root of the nostrils, using a silicone implant or finely diced autologous rib cartilage. The procedure is performed through a small incision inside the mouth, helping to avoid visible external scarring. By elevating this sunken area, the procedure softens deep nasolabial folds, reduces the appearance of a protruding mouth, and creates a more refined, three-dimensional facial impression. The incision is made inside the nostril, leaving no visible external scarring. Depending on the patient's condition and wishes, implant replacement or an autologous rhinoplasty procedure, such as septal extension, can be performed simultaneously.
