Mid-face Lift (Subperiosteal) + Upper Eyelid Sagging Removal

This patient came to us about sagging in the mid-face and heaviness of the upper eyelids. Both concerns came from the same cause: the soft tissue of the cheek had descended, and the upper lid skin had lost its support.

We lifted the mid-face in the subperiosteal plane. Working directly on the bone surface lets us reposition the cheek fat pad as one unit, so the lift holds its position instead of relaxing within a few months. At the same time we removed the excess skin of the upper eyelids.

One month after surgery the cheek sits higher and the shadow along the nasolabial fold has softened. The eyes open more easily and the face reads as more rested. Swelling at this stage is still settling, and the contour will refine further over the next two to three months.

The subperiosteal approach reaches a deeper layer than a standard thread lift or skin-only lift, so it suits patients whose sagging comes from volume descent rather than surface laxity.

Swelling and bruising are expected for the first two to three weeks and settle gradually.

Temporary numbness or tightness in the cheek and upper lid may last several weeks to a few months.

Left and right asymmetry can appear during the healing period. Most cases even out as swelling resolves.

Scars are placed inside the hairline and in the eyelid crease. They fade over time but do not disappear completely.

Infection, hematoma, and delayed wound healing are uncommon but possible.

Rarely, temporary weakness of facial expression may occur if a nerve branch is irritated during surgery.
