Excess upper lid skin had reached the lash line on both sides. Patients at this stage often describe the eyes as heavy by the end of the day, and some raise the brow constantly to compensate, which sets forehead lines.
We marked the amount of skin to remove with the patient sitting upright and the brow relaxed, then excised it and closed within the natural crease.
At one month the crease is visible, the lash line is clear, and the eyes open without effort. The forehead is no longer doing the work of holding the lids up.
The marking is the operation. Removing too little leaves the original complaint, and removing too much prevents the eye from closing.
Swelling and bruising around the eyes are strongest for one to two weeks.
Scars sit in the eyelid crease. They are red at first and fade over several months.
Temporary difficulty closing the eyes fully, dry eye, or a foreign body sensation may occur.
Asymmetry in crease height or shape can occur and may need revision.
The result changes gradually with further aging.
Infection and bleeding are uncommon.
