Patient has completed breastfeeding and presents with moderate existing subcutaneous fat in the breast area. She desires a hybrid breast augmentation combining silicone implants with fat grafting for a more natural contour and upper pole softness.
Implant: Motiva Ergonomix 275cc
incision: Inframammary fold
Pocket: Subglandular placement (under mammary gland, above muscle)
Fat grafting: Supplementary volume to be added to the cleavage and upper pole area to achieve a smooth transition and natural softness.
Donor site: Outer thighs (lateral protrusion area), where the patient strongly wishes to achieve visible contour reduction.
Liposuction will be performed thoroughly to address the lateral fullness of the thighs, with an emphasis on achieving a slimmer leg silhouette. The patient prefers not to retain unused fat, so excess harvested fat will be discarded after selecting high-quality graftable fat for the breast augmentation.
Care will be taken to ensure symmetry, natural breast movement, and minimal visible scarring.
Infection at the implant or fat-graft site — may require implant removal in severe cases
Bleeding, hematoma, or seroma – fluid or blood buildup may need drainage
Fat necrosis or nodules – transferred fat may die and form hard lumps, sometimes requiring surgical removal
Capsular contracture – scar tissue may tighten around the implant, causing pain or distortion; correction may be needed years later
Implant rupture or leakage – may go undetected without imaging; leaked silicone can cause inflammation
Rippling or visibility of implant – more likely if skin is thin or fat coverage is insufficient
Asymmetry or implant malposition – includes issues like bottoming out or double-bubble deformity
Changes in sensation – numbness or increased sensitivity in breasts or nipples
Poor wound healing or scarring – especially in patients with slower recovery or thin skin
Anesthesia-related complications – standard risks with any surgery
Rare but serious risks – blood clots (DVT), pulmonary embolism, or chest infection
Unsatisfactory cosmetic results – uneven fat retention (~30–70%), inadequate volume, or need for revision
Liposuction donor site issues – contour irregularity, skin laxity, or residual discomfort
Surgery duration: ~3 hours (includes liposuction and fat graft processing)
Initial recovery: 1–2 weeks of swelling, bruising, and discomfort (breasts and donor areas)
Full results: fat graft retention stabilizes over 3–6 months
Garment use: wear compression garments and post-op bras for 4–6 weeks
Follow-up: stitch removal after ~7 days; additional visits as needed
Restrictions: no upper-body workouts, massage, or temperature extremes for ~1 month
