The dark area under the eye is rarely pigment. Here it was a shadow cast by fat bulging against the lid with a depression sitting directly beneath it, and light falling across that step is what the eye reads as a circle. Taking the bulge down without filling the step leaves the face looking hollow instead of tired, which is why both were done in one session. The fat used for the injection was harvested from the patient and concentrated before placement, so the material belongs to the patient and integrates with the surrounding tissue. This photograph is from the end of the session. Bruising and swelling follow for two to three weeks, and the contour is judged at three months once part of the graft has resorbed.

Swelling and bruising of the lower lids are marked for the first week and can take three weeks to clear fully. The area may feel tight or numb while the tissue settles. Removing too much or too little fat is possible and shows as a residual bulge or a hollow that needs revision. Lid position can change temporarily, and in rare cases the margin turns outward and requires correction. Grafted fat is partly resorbed over the first three months, so the volume seen early is not the final volume, and lumps or small cysts occasionally form and may need treatment. Bleeding, infection, and visible scarring are uncommon. Any change in vision after surgery around the eye is investigated without delay.
