Six threads were enough in this case because the laxity sat mainly along the jaw rather than across the whole cheek, and adding more would have moved tissue that did not need moving. The vectors were angled toward the temple so the lift follows the path the tissue took as it descended. I photograph this patient obliquely as well as from the front, since a change at the jaw border is difficult to judge on a frontal image alone. At one month the threads themselves are still doing most of the work. Collagen forms along each one during the second and third months and gradually takes over that role. Facial massage and dental treatment requiring wide mouth opening are postponed until then.

Threads passed beneath the skin leave swelling and bruising around the temple and cheek for about a week, together with a pulling sensation whenever the mouth opens wide. Small puckers at the entry points are common in the first days and usually flatten without treatment. The two sides may settle at different rates, and a small correction is occasionally needed. A thread can rarely be felt through the skin, shift position, break, or become infected, in which case it is removed. Numbness or altered sensation over the cheek can persist for several weeks. Because the material is absorbed over time, the lift is not permanent and repeat placement is required to hold the result.
