B TWICE places the sutures from the posterior surface of the eyelid, so there is no incision or knot on the skin side. That keeps swelling limited and leaves no visible mark at the fold. The line looks slightly higher and firmer immediately after treatment than it will once the swelling settles, which usually takes about a week. Because nothing is placed on the anterior surface, the height can be revised more easily later than after an incisional method. The fold reaches its settled width at around four weeks.

Swelling makes the crease look higher and wider than the planned result for about a week, with bruising over a similar period, and the shape continues to refine for one to three months. Knots may be felt as small bumps and can occasionally be seen through the skin. The crease can loosen, weaken, or disappear over time, in which case the sutures are placed again. Asymmetry, irritation, and eye discomfort can occur, while infection is rare.
