This patient had lost volume in the upper breast and wanted the shape restored rather than made larger.
We chose a moderate implant size matched to her chest width and placed it under the muscle through the inframammary fold. Going too large in a slim frame makes the edge of the implant visible, so the size is set by the anatomy rather than by a cup number.
At one month the implant has begun to settle and the upper slope looks natural. The photo shows a shape that already sits well, though the final position appears at around three months.
Placement under the muscle gives better coverage over the upper edge of the implant in patients with thin breast tissue.
Pain and tightness are strongest for the first three to five days.
Swelling and bruising last two to four weeks.
The implant sits high at first and settles over two to three months.
Capsular contracture can occur months to years later and may require revision surgery.
Asymmetry, rippling, and changes in nipple sensation are possible.
Infection and hematoma are uncommon but may require additional treatment.
Implants are not lifetime devices. Replacement may become necessary in the future.
