Doctor's Comments

Two things were making this patient look tired. A hollow ran under each eye and cast a shadow, and the upper lid skin had dropped onto the lash line.

We grafted the patient’s own fat into the tear trough to fill the hollow, and removed the excess skin from the upper lids. Filling below and opening above have to be balanced. Correcting only one of them leaves the eye looking unfinished.

At one month the shadow under the eye has lifted and the upper lid shows a clean crease. The eye reads as more open and the whole face looks rested.

Grafted fat that survives the first three months stays permanently, which is the main reason we choose it over filler for this area in patients who want a lasting result.

Risks & Side Effects

Swelling and bruising around the eyes are strongest for one to two weeks.

Some of the grafted fat is absorbed over the first three months. A second session is sometimes needed.

Lumps or unevenness in the grafted area can occur and may require treatment.

Asymmetry between the two sides is possible.

Scars in the eyelid crease fade but do not disappear.

Dry eye, difficulty closing the eyes fully, or changes in the crease shape may occur temporarily.

Infection and bleeding are uncommon.
