This patient wanted more projection and a rounder upper curve without an implant.
We harvested fat from the waist and thighs, processed it by the Condense Rich method to concentrate the viable cells, and injected it in fine layers across the buttock. Taking fat from the waist at the same time narrows the line above the hip, so the shape reads better even before the graft settles.
At three months the graft has stabilised and the upper curve holds its projection. The result at this point is close to what remains long term.
Fat placed in one thick pocket does not survive well. Layering it thinly is what determines the take rate.
Swelling and bruising at both the donor and injected sites last two to four weeks.
Part of the grafted fat is absorbed over the first three months. Around 60 to 70 percent typically remains.
Sitting directly on the buttock is restricted for two to three weeks after surgery.
Firmness, lumps, or asymmetry can occur and may require correction.
Temporary numbness of the skin is common.
Fat embolism is a rare but serious complication of fat grafting to this area. We inject above the muscle layer only, which is the accepted method for reducing this risk.
Infection, seroma, and hematoma are uncommon.
