    The procedure is performed under eye drop anesthesia combined with local, relax, or IV anesthesia.

  • Procedure

    After local anesthesia, a 5–7 mm incision is made in the conjunctiva behind the lower eyelid to remove the orbital fat that causes dark circles and sagging. The incision is then sutured, and the same procedure is performed on the other eye. If only fat removal is performed, the surgery takes about 30 minutes. There are no visible scars on the face, and swelling after the procedure is minimal.

Q Can dark circles under the eyes be removed by fat reduction alone?
A

If the swelling is mild, degreasing alone can effectively reduce dark circles. However, if swelling is severe, degreasing alone may cause the skin to sag and wrinkles to form. For hollow areas in the mid-face or along the nasolabial fold, combining degreasing with fat injection is recommended. In cases of significant swelling or sagging skin, removal of excess lower eyelid skin may be necessary.

Q Will dark circles and puffiness under my eyes return after treatment?
A

Dark circles and puffiness may recur, especially if only fat removal was performed. Some patients notice a return after 2-3 years despite treatment. Multiple procedures may be necessary depending on the symptoms. At BIANCA CLINIC, we offer follow-up surgeries for those with recurrence, even if previously treated elsewhere. Fat injection can help prevent recurrence. After fat injection, we recommend avoiding excessive eye touching and significant weight loss to maintain results.

Q Where is fat harvested for under-eye fat injection?
A

Fat is typically harvested from the thighs. If there is insufficient fat due to a slim physique or recent liposuction on the thighs (within six months), fat is taken from the abdomen. The harvested fat is then purified and injected during the procedure.

Q Does the area of the thigh where fat was harvested hurt?
A

You may experience tingling or a mild muscle pain-like heaviness for about three weeks. The harvested thigh area may also feel temporarily firm. Starting two to three weeks after surgery, you’ll be advised to begin massaging the area to aid recovery.

Q How much downtime is there for fat removal or fat injection?
A

Downtime varies by individual. For fat removal alone, recovery takes about one week. If fat injection is included, downtime can extend from several days up to around 10 weeks. Swelling usually peaks 2 to 3 days post-surgery. Most swelling occurs under the eyes, so wearing a mask pulled up can help conceal it and reduce impact on daily activities. Pain generally peaks between 2 to 4 days after treatment.

Q I don’t know what type of dark circles I have.
A

Although all are called dark circles, there are three types: black (shadow), purple, and brown. Many people have a mix of these types. The most common concern at BIANCA CLINIC is under-eye puffiness. Treatment varies by type, so during your consultation, we will identify your dark circle type and recommend the best treatment accordingly.

Q Will wrinkles disappear after dark circle removal?
A

Unfortunately, eyelid surgery cannot remove wrinkles completely. Fat injections may slightly reduce some wrinkles depending on their type, but won’t eliminate them entirely. Wrinkles caused by facial muscles, like smile lines, can be improved with Botox injections.

Q From what age can I have under-eye bag removal surgery?
A

If you are under 18 years old, parental consent and the presence of a guardian may be required for certain procedures. For consultations only, you may come to the clinic alone without any paperwork. However, for surgery, you may come to the clinic alone, but a guardian must be present at the time of payment. A guardian may also attend the consultation regarding the surgery if desired.

Q Is under-eye bag removal covered by insurance?
A

Under-eye bag removal (transconjunctival fat removal) is considered a cosmetic procedure and is therefore not covered by insurance.
