  • What is Buccal Fat Removal

    Buccal fat removal is a procedure that removes the fat mass (buccal fat) located deep in the center of the cheek to address sagging, nasolabial folds, and tighten facial lines. The subcutaneous fat in the shallow part of the cheek can be reduced through small face liposuction or fat-dissolving injections, but buccal fat cannot be removed this way. Buccal fat removal targets fat masses deeper than subcutaneous fat. Those concerned about cheek prominence when smiling may be able to make their face look refreshed and smaller by removing buccal fat.

    Buccal fat removal involves making an intraoral incision to remove the fat mass, but the scar is only about 1-2cm. Since the mucous membrane inside the mouth is incised, wounds heal quickly and are not visible externally. Although it's a surgery using a scalpel, the incision area is small, so post-operative effects are minimal and downtime is usually mild. Post-operative swelling may continue for about 1 week, and bruising for 1-2 weeks, but these can be hidden with a mask and are unlikely to be noticed by others.     Once you agree to the procedure, we begin treatment. First, we mark the buccal fat collection sites and determine the removal amount based on your preferences. The procedure takes approximately 30 minutes. During the procedure, we use nitrous oxide anesthesia and local anesthesia, but intravenous anesthesia is also available upon request.

A

While there are individual differences, the procedure time is about 30 minutes.

Q When can I see the effects of buccal fat removal?
A

After downtime symptoms like swelling and bruising subside, most people tend to notice effects relatively early. While individual experiences vary, many people feel the facial line tightening effect around 6 months post-surgery.

Q Is there pain with buccal fat removal?
A

Since we use nitrous oxide anesthesia and local anesthesia during the procedure, there is almost no pain. Intravenous anesthesia is also available upon request. Days 1-2 after surgery are the peak for pain, but we prescribe pain medication as needed.

Q What should I be careful about in post-operative recovery?
A

Depending on the doctor's judgment, we may request 24-hour face band fixation after surgery. Please be careful not to brush teeth too vigorously or stimulate with the tongue around the intraoral wound. To keep the wound clean, please continue gargling with mouthwash for one week.

Please avoid carbonated drinks, spicy or hot foods, and alcohol for one week. Makeup and face washing are possible from the next day. Showering is possible on the same day, but bathing is possible from the next day.

Q Is there downtime after the procedure?

Buccal fat removal has relatively little downtime, and some people experience almost no symptoms like swelling or bruising. Swelling peaks 1-2 days after surgery. Bruising becomes almost unnoticeable within about a week. Swelling does not spread to the eyes or entire face and can be sufficiently hidden with a mask.

If you're concerned about heat sensation in your cheeks, we recommend cooling. Also, sleeping with your upper body elevated can reduce facial swelling and may help control swelling.

Q Will sagging occur again in the future?
A

Buccal fat is said to have long term effects once removed. Since fat cells themselves are removed, there is generally no concern about recurrence.

A

Removing buccal fat in your 20s-30s is said to prevent potential future cheek sagging. However, if unsuitable candidates undergo removal, there's a higher possibility of stronger aging-related sagging or hollowing. Especially for buccal fat removal at a young age, we recommend careful judgment in consultation with a doctor.

Q Can I eat immediately after the procedure?
A

Since we make an intraoral incision, please refrain from eating for 2-3 hours right after your procedure. When you start eating, avoid hot, spicy foods and foods that stick to teeth, and start with soft foods.
