Heavy upper lids were treated by removing the excess skin through an incision placed in the natural fold, where the scar sits hidden once healed. The amount removed is measured with the patient sitting upright, since lying down changes how much skin appears redundant. This image is eight months after surgery, which is unusually late for a case photograph and useful for that reason: the scar has matured and faded, and what is visible is the settled long term result rather than an early one. The eye opens more easily and the lash line is no longer covered, without the shape of the eye being altered.
Bruising and swelling of the lids are heaviest for the first three to five days and fade over about two weeks. Stitches are removed within a week and the scar sits in the crease, pink at first, then paler over six months without vanishing. Dry eyes, tearing, or difficulty closing the lids fully can occur while healing. Too little or too much skin may be taken, leaving asymmetry that needs revision. Hematoma and infection are unusual.
