The side view was the concern here. The buttock had little projection, so the line from the lower back to the thigh ran almost straight.

We took fat from the waist and abdomen and grafted it into the buttock, concentrating the volume in the upper outer quadrant. Removing fat from the waist at the same time deepens the curve above the hip, and that contrast does as much for the profile as the graft itself.

At three months the projection holds and the line from the back through the buttock to the thigh forms a continuous curve.

We assess this procedure at three months. Before that, swelling and the fat that has not yet stabilised both overstate the result.

Swelling and bruising at both the donor and injected sites last two to four weeks.

Part of the grafted fat is absorbed over the first three months. Around 60 to 70 percent typically remains.

Sitting directly on the buttock is restricted for two to three weeks after surgery.

Firmness, lumps, or asymmetry can occur and may require correction.

Temporary numbness of the skin is common.

Fat embolism is a rare but serious complication of fat grafting to this area. We inject above the muscle layer only, which is the accepted method for reducing this risk.

Infection, seroma, and hematoma are uncommon.
