The survival of grafted fat depends on how the fat is handled between harvest and injection. The BeautiFill system processes the tissue in a closed circuit, which shortens the interval and reduces handling. The fat is then placed in fine passes across several planes rather than as a single bolus, so each portion sits close to a blood supply. Fat harvested with a laser assisted system is processed and reinjected within the same operation. Not all of the transferred volume survives, and the proportion that does is settled by around three months, which is why both time points were photographed.

Both the harvest area and the breasts swell and bruise for one to two weeks, and the donor site can stay firm or slightly uneven. Pressure on the breasts is avoided while the graft settles. Part of the grafted fat does not survive, so volume decreases over three to six months and a second session may be planned, with the amount retained varying between individuals. Oil cysts, calcification, hard lumps, or infection can occur, and such changes show on mammography, so radiologists are told about the graft.
