The lower lid is the one place where adding volume often makes matters worse. Skin here is well under a millimetre thick and any material that swells will read as puffiness by the following morning, so I use collagen rather than a filler when the complaint is texture and dullness rather than a hollow. A blunt cannula was passed from a single entry point on each side to keep bruising down and to stay above the vessels. The product was laid in a thin even layer and not stacked. What you see immediately after is partly the volume of fluid placed. The real change reads at three to four weeks as the collagen integrates.
Bruising is the most common problem in this area because the vessels sit close to the surface, and it can last one to two weeks even with a cannula. Puffiness for the first few days is normal and settles. Product placed too superficially may show as a pale or lumpy line and needs massage or removal. Redness, itching and tenderness at the entry point occur and are short lived. Allergic reaction to collagen is rare but possible, which is why a history is taken first. Vascular events near the eye are very uncommon and are minimised by slow delivery through a blunt cannula. The effect is not permanent.
