Etching only reads well when the fat layer above the muscle is thin enough for the grooves to sit on real anatomy, so the lower back was included rather than treated as an optional extra. Leaving it untouched would have left a roll above the waistband that pulls the eye away from the abdomen. The harvested fat was condensed before grafting to the buttocks, which raises the proportion of viable cells and reduces the volume lost over the following months. At one week the definition looks sharper than it will be at three months, because swelling still sits between the grooves. I ask male patients to judge the result at six months, when the graft has settled and the etched lines have softened into something that reads as anatomy.

Fat grafting to the buttocks carries a risk of fat embolism if graft material enters a large vein, which is why injection stays above the muscle layer. Swelling, bruising and firm areas are expected for several weeks, and the treated skin may feel numb or itchy while nerves recover. Contour irregularity, grooves that sit unevenly, or asymmetry between the two sides can appear as swelling resolves and may need revision. Part of the grafted fat is absorbed during the first months, so buttock volume reduces from what is seen immediately after surgery. Seroma, infection, skin burn from the ultrasound probe and pigmentation at the access points are possible. Weight gain after treatment can undo the definition.
